Preparation for your visit/appointment at Northside Family Medicine

For Returning Patients- You can save time before coming to your visit by doing a couple things
1) You can now schedule your appointment online click Make appointment at Northside Family Medicine to schedule then come back to www.northsidefammed.com
2) Bring all of your medications to each visit- not just your list.
3) Bring your copay in either cash or credit card- (Visa, Mastercard, or Discover Card) as checks are not accepted.
4) Call by 8:30AM the day of your appointment if you cannot make it-you may leave a message on our voice mail.  Please review our policy on no show appointments.
5) If it has been a year or more since you signed our standard consent page for privacy policies, office policies, etc.- you should print and sign a copy of the Northside Family Medicine Signature Page if you want to review our most current policies review our Northside Family Medicine Guidelines.
6) If you have moved or changed jobs or phone numbers please fill out the Northside Family Medicine demographics.
7) Also bring a list of any recent specialists or emergency rooms that you have recently been to or currently see for any problems.
8) Arrive 15 minutes before your visit.  If you have everything filled out (always a new Review of Systems Form and any necessary updates on other forms) then arrive 5-10 minutes before your visit.

For first time visits-  You can save time before coming to your visit by doing a couple things

2) Bring the name and contact information (phone number address, etc) for your old primary care doctor.
3) Please read the Northside Family Medicine Guidelines and fill out the Northside Family Medicine new patient package. 
4) Bring all of your medication bottles if possible instead of a list of them.
5) Also bring a list of any recent specialists or emergency rooms that you have recently been to or currently see for any problems.
6) Bring a family history for your family- for your parents, grandparents, aunts, uncles, siblings, and children with any significant medical problems including cancer, breathing issues, high blood pressure, heart attacks, strokes, blood clots, depression or mental illness and any other pertinent family history.
7) Bring a list of your previous surgeries and major medical problems such as high blood pressure, diabetes, etc. 
8) Call if you cannot make your appointment by 830AM on the morning of your appointment.
9) Make sure you bring your copay either in the form of cash or credit card- Visa, Mastercard, or Discover Card as checks are not accepted.
